FFN Member Registration Form

Native American Family, Friend and Neighbor (FFN) Child Care

We are asking FFN Caregivers to complete this registration form to have access to our member portal and to help us understand the needs and preferences for supporting FFN child care. Your input helps prioritize supports and resources for the FFN system. These services are available to unlicensed child care providers who care for children of family members, friends or neighbors while their parents work or attend school. Please Note: Foster Care is not considered to be FFN child care for the purpose of these services.

Data privacy notice: We are gathering the information on this form to learn a little about you and what kind of support you are looking for, and to make sure we know the best way to communicate with you. We will use the information you share to provide you with resources and connections to meet your needs.  

 

You may skip any question below, or decline to provide us any of the requested information; we will still offer you resources that may help with your caregiving, but we may not be able to proactively communicate with you about opportunities that might be helpful to you. Additionally, the resources that we offer you may not be as personalized to your specific needs.  

 

Only staff at All Nations Rise who are working with Family, Friend, and Neighbor caregivers will see this form; we will also share the information, without your name or contact information attached, with staff at the Minnesota Department of Children, Youth and Families, which funds the Family, Friend, and Neighbor program.  The Department of Human Services will use this information to evaluate and improve the state’s support for Family, Friend, and Neighbor caregivers. The only other reason this data would be shared would be for limited exceptions authorized by law.
